Chronic Kidney Disease (CKD) is a significant public health problem with patients being at risk of increased morbidity and mortality not only as a result of end-stage kidney failure but through cardiovascular disease.
Historically the management of CKD across primary and secondary care has been fragmented, provided without the benefit of complete medical information and mainly focused on acute services within hospital and the treatment of end-stage renal failure through dialysis and transplantation.
Over the last 5 years several drivers have resulted in a sea change in the patient pathway within primary care and between primary and secondary care;
a) The introduction of eGFR reporting for the identification and monitoring of patients with chronic kidney disease.
b) NICE guidance on the early identification and management of chronic kidney disease in adults in primary and secondary care. Or you can view an abridged two page version of the guidelines.
c) The inclusion of CKD monitoring in the GP Quality & Outcome Framework (QOF) to provide an opportunity to identify, register and monitor patients with chronic kidney disease,
d) Establishment of a CKD Network across Kent
