Patient initiated follow up

We are starting to implement patient-initiated follow-up. 

Patient-initiated follow-up means you can make an appointment to see your consultant or healthcare team if you need it, rather than be given a routine follow-up appointment. 

For example, if you have a ‘flare up’, your symptoms relating to your condition worsen or if you feel that you are not recovering well, you or your carer can make an appointment to see us within a specified timeframe.

This prevents you having regular review appointments when you do not want or need them. Instead you can contact us and we will arrange for an appointment to be made when you need one. This appointment will be via telephone, video or face to face. 

If patient-initiated follow-up is appropriate for you, it will be discussed with you during your consultation.
