My Healthcare Passport

During 2011 local stakeholders came together to create My Healthcare Passport for potentially vulnerable people in east Kent. With help from East Kent Hospitals Charity, this was designed and published in 2012. 

The same year East Kent Hospitals was awarded research funding by Foundation of Nursing Studies & General Nursing Council Trust to work collaboratively with local people to implement and evaluate the Healthcare Passport. The Co-Researcher Team - consisting of East Kent Hospitals staff, people with learning disabilities, parent carers and care home manager - gathered information from local people about My Healthcare Passport and it's use.

Three themes of work emerged :- 1) Raising & sustains awareness 2) Making Improvements to the Passport 3) Using IT systems to reduce risks and making accessible. 

The Co-Researcher team are pleased to be able to publish a new updated and improved version of My Healthcare Passport, that can be downloaded, typed into and stored on computer.
