Repeated Admissions Care Pathway 

Over the last three years East Kent Hospitals has been monitoring local data that shows that people with learning disabilities are more likely to experience repeated admissions to hospital and more likely to be admitted via A&E.

Collaborative Work with Community Learning Disability Link Nurses has helped us develop the pathway for people who have been admitted three or more times or been to A&E 4 or more times in the last 12 months.

An easy to read letter is shared with the patient and carers to ensure their understanding of the reasons for making this referral.

Ward staff are encouraged to use an EKHUFT system EDN to have the referral made. Instructions for this are available here.

EKHUFT & KCHFT have found that Community Learning Disability Nurse intervention has had an impact on the rate of repeated admissions in 2013, see the presentation below. A formal review of outcomes is planned.
