The stroke specialist nurse’s role is to support the stroke survivor following discharge from hospital. They will look at medications for the patient; follow up investigations, link into rehabilitation services via the intermediate care team, providing access to Stroke Association services and local support groups, other health services and social services. The stroke survivor will always be known to the Stroke Specialist Nurse and will have the opportunity for review on discharge, at six months and yearly.

The family and carer support worker is a service that gives practical information and emotional support to the families and carers of people who have had a stroke. The service helps families, and those who live alone, prepare for the changes that inevitably happen because of a stroke and helps them to cope both practically and emotionally. This is a service available in the community, however they also provide support to each of the stroke units.
