A stroke, brain injury or other neurological event can happen out of the blue, leaving people feeling frightened, bewildered and vulnerable.  

Upon their return home from hospital, the individual and their family often requires practical and emotional support to make the necessary adjustments to their lives. Unfortunately however, trying to find this support can be difficult and frustrating.  

The aim of this information directory is to provide a single point of access to the community support that is currently available. Please email harry.goncalves@nhs.net to get in touch if you spot a gap or come across information that is out of date; this will help us to ensure that it remains as useful a resource as possible.
